We at AgeWell value our appointment time spent with you and want to make it as productive as possible. In order to make the most of your visit at AgeWell Medical Associates, we would like to discuss the types of visits that are available to you.
Acute Visit: This visit is usually for an acute problem such as swelling, chest pain, shortness of breath, fever, abdominal pain, nausea, vomiting – as a few examples. For this kind of a problem you might think about going to the Emergency Room or Urgent Care, but WE WANT TO SEE YOU FOR THESE PROBLEMS. Our responsibility to you as your primary care provider is to see you immediately – the same day – for such problems. We truly believe that your primary care provider can do a better job of evaluating such problems than someone who has never met you before. We strive to schedule such appointments with your primary provider, but if that is not possible you may be seen by another provider.
Same Day or Routine Visit: This visit would be for a problem that you may have had for awhile but would like to be seen by your provider as soon as possible. In many cases you could be seen that day but if not an urgent concern, you might be scheduled for another day. It is best to see your own provider for continuity of care.
Follow Up Visit: This visit is for a specific concern, could be for high blood pressure, diabetes, heart disease, COPD, swelling – as a few examples. This type of visit is to manage chronic medical conditions and work with you to develop a plan of care. These visits are usually at 3 – 6 – 12 month intervals. We ask that if you have a new problem or concern that you do not save these issues to discuss at your scheduled follow up visit. If that happens, your provider would most likely address the new concerns and reschedule your follow up visit.
New Patient Visit: This is a first visit to the practice. Past medical, surgical, family, social, prescription histories are all discussed in detail, as well as any current concerns. A physical examination may also be completed. We ask that you bring all medications and supplements in their original bottles to this visit. A plan of care is set up in partnership with you and a follow up visit schedule is established.
All appointment time slots are 15 – 30 – 45 – 60 – 90 minutes depending upon the type of visit. In order for the overall schedule to run smoothly and have our patients seen in a timely manner, we strive to schedule you for the appropriate type of visit and time slot. As a courtesy, our front office staff calls to remind you about your appointment 24 hours in advance. For a Monday appointment – reminder calls are done on Friday. The front office staff will leave a message and if the phone is busy – will call again to try to speak to you directly.
